State of Mind Counselling Service Agreement
- Introduction
This Service Agreement outlines the terms and conditions for counselling services and programs provided by State of Mind Counselling Services (hereafter referred to as “the Service”). It applies to counsellors and social workers, as well as to clients including young people, families, individuals, parents, and guardians. - Services Provided
The Service offers a variety of counselling sessions and programs to support clients’ mental health and well-being. This includes sessions conducted in-office, at educational institutions with prior approval, and other agreed-upon locations. - Clients and Legal Guardians
Parents and guardians, including single parents, must acknowledge their responsibility by signing this agreement. In cases of single parents, it is recognized that obtaining both parents’ signatures may not always be possible. By signing this agreement, single parents acknowledge that they have read the Privacy Agreement and accept responsibility. - Confidentiality
All personal information shared during counselling is treated with confidentiality, in accordance with the Service’s Privacy Policy and Australian privacy laws. Information is used solely for the purposes of providing effective counselling and support. For further details, please refer to the Privacy Policy provided. - Session Details
- Duration: Each counselling session typically lasts 50 minutes.
- Frequency: Session frequency will be determined collaboratively between the counsellor and the client.
- Location: Sessions will be conducted in the Service’s office or at the client’s educational institution with prior approval. Additionally, the session location can be changed with prior mutual agreement between the client and the counsellor or social worker.
- Fees and Payments
Fees: The fees for counselling services will be discussed and agreed upon prior to the commencement of services.
Payments: Payment for services must be made in agreement with the client and the counsellor or social worker. Refer to the cancellation policy for further information regarding notice and payment.
